First Appointment, Insurance and Financial Arrangements

First Appointment

Your first visit will last about an hour. Please be prepared to provide your insurance information, medical history and prescribed medication information. Our staff may mail you our comprehensive one page form, so you can complete it before you arrive for this visit.

A thorough, complete oral exam will be performed to evaluate the health of your teeth and gums.  Any necessary digital x-rays will be taken to allow us to look for cavities, impacted teeth, bone loss and oral cancer. Upon completion of the exam, we will discuss your diagnosis and treatment options.  We will provide you with a written treatment plan that outlines your proposed treatment and cost estimate.


If you have dental insurance coverage, we will do our best to use your benefits efficiently. Insurance coverage varies by carrier, so we will verify your eligibility and benefits before treatment is started. Your written treatment plan will contain an estimate of your insurance payment and your co-payment. Your estimated co-payment is due at the time of service. Dr. Swanson is a participating provider with Delta Dental Premier and United Concordia.  However, most every PPO (Preferred Provider Option) plan can be billed by our office. Insurance coverage is a contract between you and the insurance company. We will bill your insurance as a courtesy to you, but due to the complex nature of insurance contracts we cannot guarantee the payment. Any differences in your estimate and the actual payment are your responsibility.

Financial Arrangements

It is our goal to provide you with high quality, affordable dental care. Payment is due at the time of service. Our office accepts cash, checks, Visa, MasterCard American Express and Discover. For those who qualify, we have no or low interest payment plans through CareCredit financing. If you do not have insurance, we offer a 5% (10% if over 65) discount for payment in full at the beginning of treatment. We also offer a “layaway” option for payments in advance of the service. If you have any questions about financing or payments, please ask. We will work to accommodate your needs.
